Each person visiting for a foreign travel consultation will have a medical chart prepared which will include a list of the patient's chronic medial conditions, allergies, current medications, vital signs, and proposed travel itinerary. A "super bill" will be prepared for each patient for submission to their employer, sponsor, of insurance company for reimbursement, but be advised that most insurance plans will not approve or reimburse travel medical expenses. This is, of course, dependent upon your individual plan, but you must anticipate that all expenses relating to this office call, to include the visit charge, administration of injections, and costs of vaccines will be an out-of-pocket expense that will not be reimbursed.
Each member of a family will have their own chart similarly prepared. The office fee of $135 is per person. No discounts are made for multiple members of a family traveling or being see together.

There is no additional fee for immunization administration during the initial visit. Pre-arrange booster immunizations will be administered with an administration fee, but no additional office fee. The administration fee is $15.00 for each booster administered via injection. Dr Forgey's office accepts VISA, Mastercard, Discover, American Express, checks and cash.
